I’m a big scrapbooker and love to capture a little of of my kids on their last day of school. Of course this doesn’t have to be done exactly on the last day of school, any time during the first week of summer is a great time for them to sit down and fill this out (and great summer writing practice too!). If they aren’t great at reading and writing yet you can either ask them the questions and then write down what they say, or have them fill out the parts they can and you fill out the rest.
For the picture section you can either have them draw a picture of themselves, or you can put a copy of their school photo and glue it in the spot. I like to store these in the tub of all their schoolwork that I keep for the year (I sort through everything they’ve brought home during the year and pick some favorites and store them in a tuberware. This is a nice thing to have right on top! And a fun last day of school tradition.
